This role supports the coordination of home health referrals for Medicare-certified home care services. The position works closely with Home Care Representatives, referral sources, physicians' offices, and clinical staff to ensure patients transition smoothly into home health care.

Job Description:

Coordinate with physicians' offices, hospitals, clinics, and other referral sources. Review and organize referral documentation. Communicate with internal departments and external healthcare providers to facilitate continuity of patient care. Provide administrative support for office and clinic referrals as needed. Receive, review, and process patient referrals. Answer phone calls and communicate professionally with patients, providers, and healthcare facilities.

Position Requirements:

Strong customer service and communication skills. Proficiency with computers and office equipment. Knowledge of medical terminology is a plus! Excellent organizational and multitasking abilities. Ability to build positive relationships with patients, providers, and coworkers. Strong attention to detail and accuracy.
