Pay:

$40.00-$50.00 per hour, depending on applicable healthcare, intake, customer-service, and sales experience. Each completed assessment will have a guaranteed one-hour minimum.

Mileage:

Assessment Coordinators will primarily be assigned within their designated local service area. Mileage reimbursement may be available for preapproved assignments outside that service area according to company policy.

Luxe Healthcare Solutions is seeking professional, personable, and dependable PRN Client Assessment Coordinators (As Needed) to conduct in-person assessments with prospective home-care clients and their families. Assessment Coordinators represent Luxe during one of the family's first personal interactions with the company. The ideal candidate must be able to create a genuine connection, ask thoughtful questions, listen carefully, explain our non-medical home-care services, and help the family feel comfortable moving forward with care. Assessments are generally scheduled after a prospective client or responsible party has expressed interest in services. Most assessments take approximately 30-60 minutes and are completed electronically using a company-provided Google Form. After the Assessment Coordinator completes and submits the assessment, Luxe's administrative team will review the information and prepare the client's care plan.

PRIMARY RESPONSIBILITIES

Travel to prospective clients' homes, facilities, hospitals, rehabilitation centers, or other approved meeting locations. Arrive on time, dressed professionally, and prepared to represent Luxe Healthcare Solutions. Establish rapport with prospective clients, family members, responsible parties, and referral partners. Explain Luxe's non-medical home-care services clearly and accurately. Ask appropriate questions concerning the client's needs, routines, preferences, home environment, schedule, safety concerns, and desired start date. Gather complete information regarding activities of daily living and instrumental activities of daily living. Document information related to mobility, transfers, fall risks, cognition, memory concerns, meal preparation, housekeeping, transportation, medication reminders, incontinence care, companionship, personal-care needs, and caregiver preferences. Identify the primary decision-maker and responsible party. Discuss service options and next steps without making unauthorized promises regarding staffing, scheduling, pricing, or the exact start of care. Complete the entire electronic assessment accurately using Google Forms. Review the form for missing or inconsistent information before submission. Submit the completed assessment promptly, preferably before leaving the appointment or immediately afterward. Notify the administrative team when the assessment has been completed. Immediately report urgent safety concerns, unusual incidents, possible abuse or neglect, hospitalization concerns, or other significant issues to management. Protect all client and company information according to Luxe's confidentiality, privacy, and data-security policies. Maintain accurate availability and respond promptly when an assessment is offered. Participate in required orientation, assessment training, sales and communication training, and periodic quality reviews. Complete other intake-related duties within the scope of the position as assigned. This is a non-medical assessment role. Assessment Coordinators should not diagnose medical conditions, provide clinical recommendations outside their qualifications, promise caregiver availability, or independently create or approve the final care plan.

REQUIRED QUALIFICATIONS

Excellent verbal communication, listening, and relationship-building skills. Professional, warm, confident, and trustworthy presence. Ability to ask personal or sensitive questions respectfully. Strong attention to detail and accurate documentation skills. Comfortable discussing services and helping prospective clients understand the value of professional home care. Familiarity with Google Forms, email, web browsers, and basic electronic documentation. Ability to follow a standardized assessment process. Dependable transportation, a valid driver's license, and current automobile insurance. Willingness to travel throughout the assigned local service area. Ability to pass required background and employment screenings. Ability to maintain client confidentiality. Ability to provide reliable weekly availability and attend scheduled appointments consistently.

PREFERRED QUALIFICATIONS

CNA, LPN, RN, medical assistant, social-services, home-care, hospice, senior-living, case-management, admissions, intake, or healthcare-related experience. Experienced or seasoned CNAs with strong communication and documentation skills are strongly encouraged to apply. Previous consultative sales, admissions, intake, customer-service, or family-facing experience. Experience working with older adults, individuals with disabilities, dementia clients, or families arranging in-home care. Knowledge of ADLs, IADLs, mobility assistance, fall risks, dementia-related behaviors, and common home-care needs.

TECHNOLOGY REQUIREMENTS

Until company tablets are issued, candidates must have access to a reliable, password-protected smartphone, tablet, or laptop capable of completing Google Forms.

Candidates must also have:

Reliable cellular data, mobile internet, or hotspot access. The ability to access and submit online forms from the field. A secure device that is not routinely shared with other people. The willingness to follow company security requirements, including passwords and multifactor authentication when required. Client information may not be downloaded, photographed, screenshotted, forwarded, or stored outside company-approved systems.

The ideal Assessment Coordinator is someone who can walk into a family's home and make people feel heard, respected, and confident in Luxe Healthcare Solutions. This person should be friendly and conversational while still gathering complete information and maintaining professional boundaries. The candidate should be comfortable discussing services, addressing general concerns, and supporting the family's decision-making process without being aggressive or pressuring the family.

IMPORTANT POSITION EXPECTATIONS

: This is a PRN, as-needed position; regular or guaranteed hours are not promised. Assessment requests may occasionally require same-day, next-day, evening, or weekend availability. The candidate is not required to accept every offered appointment, but once an assessment is accepted, attendance is expected. Last-minute cancellations, lateness, incomplete documentation, or failure to submit an assessment promptly may affect future assignments.
