Lead. Serve. Develop. We are seeking a strong, mission-driven leader to serve as a Care Coordinator , overseeing both a caseload of veterans and a team of case managers. This role is ideal for someone who thrives in leadership, understands healthcare or case management, and wants to make a real impact serving veterans. What You'll Do Manage a caseload of veterans Ensure documentation and compliance standards are met Conduct field visits and maintain strong relationships with veterans and families Support onboarding and training of caregivers Coordinate care services Maintain strong relationships with VA and referral partners Promote accountability, professionalism, and high performance This role blends leadership, field engagement, and operational oversight.
