White County Rehabilitation and Nursing is seeking a Social Services Coordinator to support residents and families and assist with care coordination, discharge planning, and admissions.

Pay:

$18.00-$20.00 per hour

CNA:

$19.50-$23.22 per hour Schedule Full-Time Monday-Friday Responsibilities Provide support and assistance to residents and their families Address resident and family concerns and help coordinate appropriate services Participate in care planning and interdisciplinary team meetings Assist with discharge planning and coordination of outside services Maintain accurate and timely documentation Assist with admissions and intake processes Coordinate with hospitals, referral sources, case managers, and internal departments Support a smooth transition for residents entering and leaving the facility Qualifications Experience in social services, skilled nursing, long-term care, or healthcare preferred Experience with admissions or discharge planning preferred CNA certification preferred Bachelor's degree in Social Work or a related field preferred Strong communication, organizational, and interpersonal skills Ability to work effectively with residents, families, and an interdisciplinary team Benefits Medical, dental, and vision insurance 401(k) Paid time off Supportive team environment
