Job Overview:

We are seeking licensed social workers (LSW, LCSW, LICSW) to join our team. The ideal candidate will possess a strong background in medical social work and a passion for supporting clients and residents. Our social workers play a crucial role in supporting our clients through various challenges such as temporary staffing vacancies, significant administrative changes and DPH survey preparation. S2 Social Work offers flexibility, supervision and a positive, solution focused work environment.

Duties:

• Conduct comprehensive patient assessments to determine short and long term needs and care plan accordingly.
• Maintain accurate medical records and documentation in accordance with legal and ethical standards. Ensure compliance with HIPAA regulations.
• Collaborate with multidisciplinary teams to coordinate care for clients and ensure effective communication
• Complete ongoing documentation and adjust care/treatment plans as necessary based on ongoing assessments.
• Educate clients and families about community services available, mental health issues, and coping strategies

Skills:

• Ability to effectively communicate and coordinate amongst a variety of disciplines
• Experience working in a healthcare setting, proficient in medical records management and documentation standards.
• Solid understanding of social work principles and ethics.
• Ability to conduct thorough patient assessments effectively.
• Knowledge of HIPAA regulations to ensure client confidentiality and data security.
