Case Manager Full-Time | Monday-Friday | Rotating Weekends Make a Meaningful Impact Every Day Are you passionate about helping patients navigate their recovery journey and achieve the best possible outcomes? We're seeking a dedicated Case Manager to join a collaborative inpatient rehabilitation team where you'll play a critical role in coordinating care, guiding discharge planning, and advocating for patients every step of the way. This is an opportunity to combine your clinical expertise with care coordination in a supportive environment focused on helping patients regain independence and improve their quality of life. What You'll Do Coordinate patient care from admission through discharge Partner with physicians, therapists, nurses, and families to develop individualized care plans Lead interdisciplinary team conferences and treatment plan discussions Facilitate discharge planning and connect patients with community resources Educate patients and families on rehabilitation goals and expectations Monitor patient progress, insurance requirements, and care outcomes Ensure timely and accurate documentation and compliance with regulatory standards Support utilization review and quality improvement initiatives Qualifications Active RN, Social Work, PT, OT, ST, Rehabilitation Counseling, or other qualifying healthcare license Associate Degree in Nursing required for RNs (BSN preferred) Bachelor's degree required for other eligible disciplines Rehabilitation experience preferred Eligible to obtain CCM or ACM certification within two years of hire Why Join Us?

Pay Details:
