The MHCC supports children and families with complex medical, behavioral health, social, or care coordination needs. Working closely with pediatric providers, clinical staff, families, specialists, and community resources, the MHCC helps families navigate the healthcare system, overcome barriers to care, and connect with needed services and support. This role is ideal for someone who is compassionate, resourceful, highly organized, and proactive , with strong judgment and follow-through. Schedule Full-Time, 40 hours/week Weekday/daytime schedule No regularly scheduled weekends In-person at our Woburn location $25-$30 hourly Compensation is based on factors such as market pay structures, business needs, internal equity and market conditions. Individual pay is determined based on relevant experience, skills and qualifications. What You'll Do Partner with pediatric providers and clinical staff to identify and support patients and families with complex needs Develop and support individualized care plans in collaboration with families and the care team Help families navigate healthcare services and connect with specialists, behavioral health providers, schools, community agencies, and other resources Identify social, economic, and other barriers to care and help families access appropriate support Coordinate follow-up after hospitalizations, emergency department visits, specialist care, and other transitions of care Facilitate communication among families, our clinical team, outside providers, and community organizations Promote patient and family engagement, self-advocacy, and continuity of care Maintain accurate, timely documentation and follow through on referrals and identified needs What You'll Bring Education & Experience Associate or bachelor's degree, preferably in healthcare, human services, social services, or a related field Relevant pediatric healthcare experience may be considered in lieu of the education requirement 2+ years of healthcare, care coordination, case management, or related experience preferred Pediatric or family-centered healthcare experience strongly preferred Experience with electronic medical records required; Epic experience preferred Familiarity with the Patient-Centered Medical Home model and community resources is a plus Skills & Competencies Compassionate, family-centered approach to communication and problem solving Excellent verbal, written, and interpersonal communication skills Strong judgment with the ability to independently assess needs, identify next steps, and follow through Resourceful, proactive, and comfortable navigating barriers to care Highly organized with the ability to manage multiple patients, priorities, and follow-up needs Comfortable handling sensitive or challenging conversations with professionalism and empathy Collaborative approach and ability to build effective relationships with families, colleagues, providers, and community partners Strong sense of ownership, accountability, and attention to detail Why Join WPA Competitive pay aligned with experience Comprehensive benefits package Supportive, collaborative team environment Opportunity to make a meaningful difference for children and families Physician-owned pediatric primary care practice Affiliated with Boston Children's Hospital through the Pediatric Physicians' Organization at Children's (PPOC) Proudly serving generations of families in our communities for over 58 years About Us Woburn Pediatric Associates, LLP includes Woburn Pediatric Associates, North Andover Pediatric Associates , and our behavioral health practice, Woburn Pediatric Psychological Services .
