This is a full-time, hourly position, reporting to the Coordinator of Community Support Services or their designee. Care Coordinators connect clients to appropriate community resources and monitor/evaluate outcomes. This includes meeting with a case load of up to 20 clients each month, to evaluate concerns, needs and psychiatric emergencies as needed.
KEY JOB RESPONSIBILITIES
Maintain an active case load of a maximum of 20 clients per month; 15 billable hours per week Conduct a minimum of three face to face meetings per month with each client. Complete documentation and maintain files per COMAR regulations within allotted time frames. Conduct intakes, comprehensive assessments, and re-assessments Develop Plan of Care (POC) that identifies goals and interventions. Provide supportive counseling and advocacy for clients; engage clients in the problem-solving process. Advocate on the behalf of the client with external agencies, request supporting documentation from therapists, schools, doctors, or other agencies Complete a minimum of 40 hours of training in the area of case management or topics related to client care/self-care.
EDUCATION AND WORK EXPERIENCE REQUIREMENTS BA/BS
degree in a human service field. Ability to handle stressful situations. Understanding of cultural diversity. Ability to observe and accurately report the functioning of children and families.
