Job Description

Morrison Center is seeking Adult Care Coordinators for our Oakland, Maine location. Candidates should be dedicated, compassionate professionals who are passionate about empowering adults with behavioral health, intellectual and developmental disabilities. Experience working with Behavioral Health Home Services is preferred. •$1,000 sign-on bonus to be paid with first paycheck.• Benefits of the position:

Adult Care Coordinator:
Bachelor's degree in a related field MHRT/C eligible A minimum 1 year of paid experience working with individuals with behavioral health, intellectual and/or developmental disabilities (e.g., DSP, BHP, RBT, etc.) Strong organizational, writing, and time management skills Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities Previous experience in care coordination is preferred but not required Microsoft Office Suite Benefits of the position: Potential for some Hybrid work Health, Dental and Vision insurance with employer contributions Short-and Long-Term Disability coverage 12 paid Holidays Paid Time Off & Sick Time Life Insurance Employee Assistance Program (EAP) 403(b) retirement plan with employer contribution Responsibilities of the
Adult Care Coordinator:
Maintain an active caseload and monthly contact with members. Conduct home visits, community visits, and office-based appointments as needed to support member engagement and service delivery. Lead interdisciplinary team meetings and coordinate communication among providers, supports, and community partners. Provide comprehensive care coordination services to adults enrolled in the Behavioral Health Home program. Connect members with healthcare, housing, employment, benefits, transportation, and community resources. Monitor member progress and adjust care plans as needed. Complete comprehensive, person-centered, annual, and 90-day assessments in compliance with DHHS and Behavioral Health Home requirements. Promote informed decision-making, self-advocacy, and independence. Identify service gaps and collaborate with partners to address barriers to care. Maintain accurate, timely documentation in compliance with DHHS, MaineCare, Behavioral Health Home, and Morrison Center requirements. Participate in quality improvement, audit, and compliance initiatives. Serve as a liaison among members, families, providers, and community agencies. Travel within the community to support members, attend appointments, and coordinate services.
