Full-Time 40 hours/week | Monday-Friday 8am-4:30pm |

As the Social Work Assistant / Managed Care Coordinator , you will support the social services and care coordination process for residents in a skilled nursing setting. In this role, you'll work closely with licensed social workers, the interdisciplinary team, residents, families, and insurance providers to help ensure resident needs are documented, care is coordinated, and discharge planning is organized. This position combines social services support, managed care insurance coordination, documentation, and administrative follow-up. The ideal candidate is detail-oriented, organized, comfortable communicating with families and healthcare partners, and able to manage multiple timelines, records, and follow-up tasks in a fast-paced care environment. 50 / hour Key Responsibilities Assist with scheduling and coordinating care plan conferences with residents, families, and interdisciplinary team members Support discharge planning by coordinating appointments, paperwork, follow-up calls, and related documentation Coordinate in-house ancillary services, including communication, paperwork, and follow-up Submit insurance reviews, updates, and authorization requests through managed care portals or case managers Track review dates, authorizations, and required insurance notifications Serve as a liaison between the interdisciplinary team, insurance providers, residents, and families Assist licensed social workers with documentation, resident assessments, tracking records, and MDS-related support Maintain accurate logs, flow sheets, records, and department tracking systems Communicate professionally with internal departments, external partners, insurance representatives, residents, and families Provide administrative support to help ensure smooth communication, timely follow-up, and continuity of care The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform. Skills, Knowledge and Expertise Required High School Diploma or GED Must have a minimum of 2 years of supervised experience in a health care setting working directly with individuals. Ability to communicate with residents, families, interdisciplinary team, various physicians, and community organizations in a professional manner. Position requires the ability to lift and move objects weighing up to 35 pounds without assistance.
