The primary objective is to coordinate and manage the safe and efficient discharge of patients from hospitals to their next phase of care, which may include home care, rehabilitation, or long-term care facilities. Working closely with medical staff, patients, and their families, the Discharge Planner ensures that each patient's medical and psychosocial needs are met post-discharge.
