Full-Time About Quinlan Home Quinlan Home is a licensed 58-bed Boarding Care facility serving adult men and women with disabling conditions, including mental health conditions, substance use disorders, and other physical or behavioral health needs. We provide a safe, stable, and supportive place to live while helping residents maintain their independence, access needed services, and stay connected to the care and resources that support their overall well-being. Position Overview We are looking for a Service Coordinator who is organized, compassionate, resourceful, and comfortable working directly with residents while helping keep services coordinated and the facility running smoothly. The ideal person is dependable, able to manage multiple needs at once, follows through consistently, and communicates effectively with residents, staff, and outside providers. This position also supports the daily operations of the facility by monitoring resident well-being, addressing concerns, ensuring supportive services are being provided and properly documented, following up on identified needs, and assisting with other resident and facility needs as they arise. Responsibilities Assist residents with completing applications, accessing benefits, and navigating community resources and support services. Identify client needs and connect clients with appropriate outside providers, including mental health, physical health, medication management, housing, financial, and substance use resources. Clearly communicate resident concerns, changes, and necessary follow-up to staff and leadership to ensure needs are addressed promptly and appropriately. Help ensure clients receive supportive services related to basic living skills, social skills, household management, overall well-being, and problem solving. Monitor client needs, behaviors, and overall well-being and address concerns appropriately. Help ensure basic health and medication-related supports are completed and documented according to facility procedures. Complete room inspections and administer urine drug screens (UAs). Reinforce house expectations and address concerning behaviors with clients. Ensure client services, interactions, and other required information are documented appropriately and efficiently. Host monthly resident meetings to gather feedback, address concerns, and collaborate on resident suggestions that support a positive living environment and overall morale. Assist with the general day-to-day operations of the home Other duties as assigned. What We're Looking For Experience in human services, behavioral health, supportive housing, case management, substance use services, or a related setting preferred Experience with Medical Assistance and community resources strongly preferred Strong organization, communication, and follow-through Comfortable working directly with clients and addressing concerns when needed Good problem-solving skills and ability to take initiative Strong documentation skills and attention to detail Ability to maintain professional boundaries and confidentiality Reliable and comfortable working both independently and as part of a team Why Join Quinlan Home?
