(antiretroviral therapy), and medication adherence. Work collaboratively with healthcare providers, public health departments, and community-based organizations. Provide HIV, STI and HCV testing, results and linkage to care for patients/clients. Conduct outreach and education on PrEP to at-risk individuals and communities. Provide culturally competent counseling on PrEP, HIV transmission, sexual health, and harm reduction. Assist with scheduling and coordinating PrEP initiation visits, follow-ups, and lab appointments. Collaborate with DIS (Disease Intervention Specialist), community based organizations, private medical providers and local health departments for linkage, and re-linkage to prevention services. Assissit with outreach to PrEP patients who missed appointments, prior to upcoming appointments and those who are out of care. Accurately input data and complete required monthly reporting. Possess excellent written and verbal communications
Bachelor's degree in Public Health or health related field or at least 3 years work experience; or Master's degree in Public Health or a related field.
