Job Description:

Targeted Care Specialists play a vital role in supporting individuals in the community as they navigate access to resources and engage in skills work that will improve their quality of life! Our Targeted Care Specialists provide more than case management. They develop meaningful relationships with their clients while empowering them to overcome barriers and create lasting change. We are seeking a Bachelor-level candidate to work on a dynamic outreach team. Targeted care specialists support individuals with primary diagnoses of: Borderline Personality Disorder, PTSD, Major Depressive Disorder, Autism Spectrum Disorder, and other mood disorders. Specialists carry a caseload of approximately 20-25 clients they see one to two times per week until client goals are met. Must possess a valid driver's license and automobile with liability insurance in the amount required by agency policy. Basic fundamental computer experience to process e-mail and client access is required.
Requirements:
Key Responsibilities:
Works with clients to accomplish tasks (such as social exposure tasks, access to community resources, applying for benefits, and securing housing) Following through with clinical treatment through the successful use of skills (such as interpersonal, DBT, anger management, problem-solving, and self-efficacy). Work closely with clinicians on a multidisciplinary team which requires critical thinking and strong organizational abilities Staff must complete progress notes on all clients seen and attend treatment team meetings and supervisions.
Education:
The position requires a bachelor's degree in psychology, social work, rehab counseling or education/counseling, or related field from an accredited college or university.
Qualifications:
Must possess a valid driver's license and automobile with liability insurance in the amount required by agency policy Fundamental computer experience to process e-mail and client access is required. Fundamental computer experience to process email and access client/claims information within an EHR and web-based insurance portals Experience with Microsoft Word and Excel is a plus Supervisory/Management experience Job type:
