LCSW - Discharge Planning

Job Description

Nurses Etc . is seeking an experienced Licensed Clinical Social Worker (Discharge Planner) to support patients and families at Nellis Air Force Base . The ideal candidate has hospital-based discharge planning experience and is comfortable working in a fast-paced, multidisciplinary clinical environment. Qualifications Bachelor's Degree in Social Work (BSW) from a CSWE-accredited program. 2+ years of social work experience, including at least 1 year of hospital discharge/transitional care planning within the past 3 years. Current, active, unrestricted Social Work license from any U.S. state, territory, or
D.C. BLS
required . Hospital experience in ER, Critical/Cardiac Care, or Multi-Specialty Units required. Experience with discharge planning, crisis intervention, referrals, community resources, and multidisciplinary care teams. Knowledge of Medicare, Medicaid, TRICARE, Health Net, and other payor resources preferred. Strong communication, documentation, organization, and problem-solving skills. Ability to work effectively in a fast-paced, high-stress clinical environment. Key Duties Assess patients' psychosocial, financial, and discharge needs. Develop and coordinate safe discharge and transition plans. Identify and resolve barriers to discharge and placement. Coordinate referrals, community resources, transportation, DME, home health, and post-acute services. Coordinate patient transfers to appropriate facilities and services. Provide crisis intervention, counseling, advocacy, and emotional support. Collaborate with physicians, nurses, case managers, and multidisciplinary teams. Assist patients with available government and community assistance programs. Maintain accurate and timely documentation in the EHR. Participate in patient care conferences and ongoing discharge planning. Maintain patient confidentiality and comply with hospital policies and safety requirements. Preferred ASWB or NASW certification is desirable but not required.
Hours Available:
Shifts may vary from 8• 10•or 12-hour shifts between the hours of 0630-1800
Duty Hours:
Shifts may vary from 8• 10•or 12-hour shifts between the hours of 0630-1800 dependent on the mission needs not to exceed 80 hours in 2 weeks, which may include some weekends.
