Using care coordination, motivational interviewing, and health promotion, you'll be part of an outcome-driven, integrated team supporting individuals living with chronic health conditions and behavioral health challenges.

A copy of your resume Professional attire recommended References ( 2 Supervisors and 1 Co Worker) Be prepared for an on-site interview We are currently hiring for the following positions : Care Coordinator Join Our Health Home Program as a Care Coordinator!Essential Position Functions :
Work with the population served by the Health Homes Program, which includes adults living with serious mental illness, adults with chronic medical conditions, and/or individuals with a history of alcoholism and/or substance use disorders. Conduct initial assessments and reassessments of members' needs, including medical, mental health, substance use, financial, housing, and support needs. Develop person-centered care plans with documented input and approval from the member and other providers, in adherence with Health Home standards. Work with members' medical and behavioral health providers to develop, implement, and coordinate care plans for clients with chronic illnesses, such as diabetes, asthma, congestive heart failure, hypertension, behavioral health conditions, and HIV, among other illnesses, based on Health Home chronic disease care coordination model standards. Coordinate member services with all service providers through regular case conferencing in accordance with Health Home guidelines. Conduct field visits with individuals in their homes or at locations of their choice to provide services. Be comfortable conducting fieldwork in various settings, including, but not limited to, shelters, SROs, substance use treatment programs, mental health facilities, and hospitals. Respond to questions and concerns and contact other care providers and community resources to ensure that individuals receive the care and services they need. Document contacts, attempted contacts, and all other required documentation within established time frames. Attend staff meetings, supervision, conferences, and training, as required. Adhere to all approved Bridge personnel and Health Home policies and procedures. Perform other duties as assigned.Required Knowledge, Skills, and Abilities:
B.A./B.S.-level professional with at least two years of experience working with the mentally ill/substance use disorder population. A minimum of two years of experience in a mental health setting is required. Experience with electronic record systems is strongly desired. Willingness to travel via public transportation along with the ability to manage time effectively and meet program and regulatory requirements, is essential. Strong documentation skills and exceptional attention to detail. Must be willing and able to work in the field and conduct field-based services. Must have experience in outreach, client engagement, assessment, direct service and treatment, and documentation. Bilingual English/Spanish skills are a plus.
