Job Description

Position Overview Touching Hearts at Home is seeking an energetic, compassionate, and organized Client Care Coordinator to join our team. This is a full-time, 40-hour-per-week position that combines office-based work with community and client-home visits. Our mission is to empower older adults to age safely and comfortably at home while helping them avoid unnecessary hospitalization. Position Purpose The Client Care Coordinator oversees client care and services, ensuring quality, continuity of care, and a high level of satisfaction for both clients and care partners. Primary Responsibilities Monitor client needs, changes in condition, and service plans to ensure care remains appropriate and up to date. Ensure care partners understand and follow current client service plans. Conduct client home visits to evaluate the quality of care and services provided. Participate in the initial visit with potential clients and their family members to assess care needs. Develop and update client care service plans and communicate changes to families, care partners, geriatric care managers, community partners, and other members of the care team. Collaborate with the Scheduling Team to match care partners with clients. Complete introductions between new care partners and their clients. Deliver care notes, gloves, and other supplies to clients as needed. Support daily operations as needed, including on-call coverage, scheduling, clerical duties, answering phones, and providing care partner support. Perform other duties as assigned. Qualifications Associate's degree in human services or related field 1-3 years of experience working with older adults in care coordination, discharge planning, social work, home care, or a related field. Strong communication, relationship-building, and organizational skills. Ability to work independently, exercise sound judgment, and manage multiple priorities. Passion for providing compassionate, high-quality service to older adults.
