Job Description

A client care coordinator acts as a liaison between clients and our team of clinicians, providing administrative and logistical support while ensuring client needs are met through care planning, resource coordination, and communication. Key responsibilities include client intake, scheduling appointments, patient advocacy, and monitoring progress, often requiring a combination of strong communication, organizational, and problem-solving skills.
Key Responsibilities Client Intake & Management:
Handle the initial intake of new clients, manage their accounts, and provide general administrative support.
Care Coordination:
Develop and coordinate client care plans, connect clients with necessary medical appointments, therapy, and support groups.
Client Support & Communication:
Serve as a primary point of contact for clients, respond to inquiries, and provide one-on-one support to help them navigate their care.
Advocacy:
Advocate on behalf of clients to ensure their needs are met within the healthcare system.
Resource Linkage:
Link clients to relevant community programs and other resources that can aid their recovery and well-being.
Progress Monitoring:
Monitor and evaluate client progress, and collaborate on necessary interventions to improve their health outcomes.
