Make a Meaningful Difference in Your Community As a Community Care Coordinator , you'll help individuals and families overcome the social, medical, and practical barriers that can make it difficult to access health care and other essential services. This is a relationship-driven role for someone who enjoys connecting with people, solving problems, navigating resources, and advocating for individuals and families. You'll work alongside clients, health care providers, community organizations, and other partners to help people access the support they need to thrive. What You'll Do Connect People to Care and Resources Coordinate and provide safe, timely, effective, efficient, equitable, and client-centered services. Manage an active caseload of approximately 25-35 clients , building supportive relationships and helping each person work toward their individual goals. Coordinate referrals and follow up with community organizations and service providers based on each client's identified needs and priorities. Advocate for clients and assist with applications for health, social, and community-based services. Help clients navigate systems and remove barriers that may prevent them from accessing care. Build Community Connections Work directly with community organizations and service providers to improve access to essential resources. Assist clients with transportation to appointments and other services when needed. Conduct home visits as appropriate. Participate in community, interagency, and coalition meetings and develop strong, collaborative relationships with community partners. Identify gaps and barriers within health and social service systems and document opportunities for improvement. Educate and Empower Provide relevant health and wellness education during interactions with clients. Help clients develop self-advocacy skills so they can confidently navigate health care and community resources. Meet clients where they are, respecting their experiences, priorities, cultures, and individual needs. Document and Collaborate Maintain timely, accurate, and complete documentation in the designated electronic service documentation system. Enter client and provider information according to program protocols. Complete required productivity and program reports. Attend team meetings, trainings, and other job-related meetings. Complete training required by the agency, accrediting bodies, and funders. Adhere to professional standards, protocols, rules, and regulations. Work occasional irregular hours as needed and perform other related duties as assigned. Required Knowledge, Skills & Abilities Ability to learn and apply case management principles, health care management practices, and reimbursement requirements. Strong organizational and time-management skills. Excellent verbal and written communication skills. Ability to build rapport and work effectively with people from diverse backgrounds and cultures. Strong problem-solving skills and the ability to manage multiple priorities. Ability to use technology, including electronic service documentation systems. A collaborative, client-centered approach to problem solving. Minimum Qualifications High school diploma or GED. At least 1 year of experience in a public health and/or social services setting. Ohio Community Health Worker (CHW) certificate required within 12 months of employment. Valid driver's license and current automobile insurance required.
