PrimaryOne Health is currently seeking a RN Clinical Care Coordinator for our Columbus health centers.
Essential Responsibilities:
Develops a plan of care utilizing clinical quality indicators for all identified patients on case load, including but not limited to patients with diabetes, hypertension, Coumadin management, complex and comorbidity, OB/GYN, etc. Provides case management, care coordination, and patient education to ensure continuous and comprehensive care for patients on case load. Regularly evaluates plans of care with patients and providers, and care team; updates as needed, develops mutual agreed upon goals, and provide patient education and behavior change coaching. Works collaboratively with the health care team including the providers, pharmacists, dieticians, behavioral health specialist, other nursing staff and medical assistants. Develops plan of care for hospital transitions in order to identify factors to decrease the likelihood of readmission. Ensures case management is implemented and managed to improve continuity and quality of care. Works in partnership with the provider teams. Maintains communication with patients on case load concerning treatment progress and diagnostic reports. Identifies and coordinates referrals to community resources in regards to patient need. Completes documentation of all professional activities, contacts and observations promptly and appropriately. Attends internal and community meetings as assigned and participates in Quality Improvement activities. In-depth assessment and triage of more complex patients as identified by the provider group. Engages patients to take an active role in maintaining their physical and mental health. Conducts 1:1, or group education classes whenever needed. Regular travel required. Other duties as assigned. Successful candidates must be licensed as a registered nurse in the state of Ohio, BSN strongly preferred. Two to three years of clinical care experience. Hospital based nursing preferred. Background in medical-surgical nursing, working with patients with chronic or complex health issues, and case management experience strongly preferred. Current BLS required.
