Job Description
Company Overview Full Circle Recovery Services is an outpatient agency dedicated to making a significant difference in Dayton and surrounding communities by providing individualized treatment for those struggling with addiction. Our mission is to work collaboratively with clients to overcome challenges and reduce the stigma associated with addiction. Summary We are seeking a compassionate and skilled Case Manager to join our team at Full Circle Recovery Services in Dayton. This role is crucial in supporting our clients on their journey to sobriety, helping them navigate their treatment plans, and ensuring they receive the necessary resources for recovery. The Case Manager will play an integral part in our mission to provide exceptional care and support. Responsibilities Connect patients with needed and available community resources. Follow-up with clients and agencies as appropriate to document the use/success of the referral. Complete detailed reports on needs assessments and case management activities. Input accurate and complete case management data into the electronic patient database. Ensure that all documents submitted on behalf of a patient are valid and authentic. Assist patients in accessing community-based resources and acquiring the appropriate medical, social, educational, and employment resources. Connect patients with support groups and additional behavioral health resources. Assist patients in securing temporary and long-term housing. Assis in scheduling treatment appointments for patients and explain the treatment timeline and objectives. Provide crisis interventions and de-escalation techniques. Monitor and ensure that patients are seeing other healthcare professionals within and outside of Full Circle as required. Assure that documentation is completed in compliance with Full Circle requirements, regulatory agency, and best clinical practices. Coordinate patient care with the treatment team including clinical, medical staff, sober living, and other entities. Adhere to Full Circle policies and procedures, and professional code of ethics. Complete documentation utilizing the agency's designated Electronic Medical Record System, within a specified timeframe. Attend required meetings including but not limited to, individual and group supervision, treatment team meeting, and in-service training. Maintain current license and/or certifications as specified by Full Circle and credentialing boards (Ohio Chemical Dependency Professional Board). May provide various other services as needed including but not limited to group orientation and counseling. Qualifications High School Diploma or GED Must have an active CDCA License Must possess basic computer literacy skills. Must be knowledgeable of social services practices and procedures. Detail oriented and able to work in a fast-paced environment. Strong communication skills. Basic computer skills to access and utilize the EMR. A minimum of 1-year experience in mental health and/or addiction preferred but not required. This position may require the use of a Company or personal vehicle. Candidates must have a valid driver's license.
