Axis HealthCare is looking for someone to join our chronic care management team! This role is more than just taking care of patients, it's speaking life and encouragement to the patients that you talk to. We are a christian company and part of this job is showing Christ's love to His patients. The majority of this role would be spent on the phone with patients. While on the phone you would be doing the following: following up on their chronic conditions seeing how they are doing checking on referrals for them if they need a provider visit meeting their needs going above and beyond for all their care following up on hospital visits If you are interested in this position, please reply to this ad with your resume.
