Home Care Administrator/Director Position Summary We are seeking an organized, dependable, and highly motivated Home Care Care Coordinator to support the daily operations of our Home Care department. The Care Coordinator serves as a primary point of communication between clients, caregivers, families, and the Home Care Administrator. This position is responsible for coordinating caregiver schedules, monitoring Electronic Visit Verification (EVV), maintaining accurate client and employee information, assisting with authorizations, addressing staffing concerns, and ensuring services are delivered according to each client's approved plan and schedule. The ideal candidate is able to work in a fast-paced environment, communicate professionally, manage multiple priorities, and resolve day-to-day staffing and service issues efficiently. Key Responsibilities Coordinate and maintain caregiver schedules based on client needs and authorized service hours. Match caregivers with clients based on availability, qualifications, location, and client needs. Monitor daily caregiver clock-ins, clock-outs, missed visits, late visits, and EVV exceptions. Follow up promptly on missed or incomplete EVV documentation. Assist with maintaining EVV compliance and accurate visit verification. Monitor client authorizations, approved hours, service dates, and authorization expirations. Notify the Home Care Administrator of authorization, scheduling, staffing, or compliance concerns. Communicate schedule changes, call-offs, and coverage arrangements with clients, families, and caregivers. Assist with finding replacement coverage for caregiver call-offs and open shifts. Maintain accurate caregiver and client records within agency systems. Assist with caregiver onboarding, documentation collection, and readiness for assignment. Ensure caregivers are scheduled only within their approved scope, availability, and client authorization. Communicate professionally with service coordinators, clients, families, caregivers, and internal departments as appropriate. Document client, caregiver, and scheduling communications accurately. Report client complaints, incidents, hospitalization notifications, changes in condition, or other significant concerns immediately to the Home Care Administrator. Assist with audits, compliance reviews, and preparation of Home Care documentation. Maintain confidentiality and follow HIPAA and agency policies. Participate in staff meetings, training, and quality-improvement activities. Provide administrative and operational support to the Home Care Administrator. Perform additional Home Care coordination duties as assigned. Qualifications High school diploma or equivalent required. Previous home care, healthcare, scheduling, or care coordination experience preferred. Experience with Electronic Visit Verification (EVV) systems preferred. HHAeXchange experience is a plus. Strong computer and data-entry skills. Excellent written and verbal communication skills. Strong organizational and time-management abilities. Ability to manage multiple schedules and priorities simultaneously. Ability to handle confidential information professionally. Strong problem-solving skills and attention to detail. Must be dependable and able to work independently while following established department procedures. Preferred Experience Experience with any of the following is highly desirable: Pennsylvania Medicaid Home Care Personal Assistance Services (PAS) Community HealthChoices (CHC) Home and Community-Based Services Managed Care Organizations (MCOs) EVV compliance HHAeXchange Caregiver scheduling and staffing Client authorizations Home Care compliance and documentation What We're Looking For We are looking for someone who understands that successful Home Care coordination requires more than filling shifts. The Care Coordinator must be proactive, responsive, detail-oriented, and committed to ensuring clients receive their authorized services while caregivers and agency records remain compliant. The successful candidate will be comfortable communicating with clients and caregivers throughout the day, resolving scheduling problems, monitoring EVV, identifying potential compliance issues, and escalating concerns appropriately. $20.00 - $22.00 per hour
