SUMMARY OF JOB

Social Services coordinator position identifies the needs of each resident/patient/families and assures that the needs are met by the appropriate discipline. Assists patients/residents/families in improving /maintaining and their ability to manage their physical, mental and psychosocial needs.

RECRUITMENT REQUIREMENTS

BSW or BA in sociology/social work or related human services/healthcare field Minimum one year experience in a long-term care setting or equivalent related healthcare experience Excellent oral and written communication skills Knowledge of community resources Experience working as part of an interdisciplinary team Computer experience
