Job Description

Job Summary Accurately bases observations and documentation on identified plan of care. Actively participates in the provision of a safe and therapeutic millieu. Actively participates in multi-disciplinary treatment team meetings (Master Plan and Reviews). Appropriately adapts to change in a positive, professional manner. Communicates effectively. Establishes and maintains two-way communication with peers, staff, physicians and administration. Communicates with Unit Director on unit/patient care issues for appropriate follow up. Competently responds to patient crises with appropriate behavior management skills. Consistently documents legibly, accurately and in a timely manner according to guidelines. Exercises therapeutic communication skills with patients. Follows established program schedules and continuously communicates with other disciplines to ensure a safe therapeutic environment and follows through on problems that may compromise patient care. Effectively utilizes conflict management and de-escalation skills as necessary with patients. Represents the hospital and program professionally and positively in the community. Willingly and safely transports patients to and from facility at request of coordinator/designee. Correctly identifies and uses appropriate personal protective equipment. Assists in designing topic- specific didactic group sessions focusing on topics pertinent to the patient population. Consistently assists Mental Health Counselors to conduct psycho-educational groups according to the treatment schedule. Assists in the orientation and mentoring of new and current Mental Health Workers. Demonstrates clinical competency. Follow Psychiatric Resource Partners Time and Attendance policy. Identifies individual learning needs and pursues opportunities for professional enrichment by attending continuing education programs/in-services. Participate in treatment planning by helping to identify patients problems, needs and strengths. Report all areas of concern regarding patients mental or physical status to the Charge Nurse. Plan and prepare adequately for supportive group activities. Provides age specific care for the patient/family population.
