Position Overview:
The Care Ambassador supports Resource Navigators in providing coordinated services to patients/clients, caregivers and their families through the connection to pertinent resources and supports. This role focuses on improving client experience, improving outcomes and decreasing rehospitalizations by addressing issues related to social determinants of health and other barriers that may impact success in the community.
Essential General Responsibilities & Expectations:
- Maintain a positive attitude and communicate professionally with staff, clients/patients, caregivers, family members and other professionals.
- Work as a team player to coordinate care along our spectrum of departments and services to ensure that each patient receives the right care at the right time
- Ensure compliance with company guidelines, professional standards and regulatory requirements
- Maintain regular contact with assigned patients/clients, caregivers and families through calls, messages, or visits as directed
- Prepare, organize, and maintain accurate, timely, and confidential notes and records in electronic and/or paper formats.
- Participate in interdisciplinary team meetings, case conferences, in services and education as assigned or delegated .
- Support outreach events, education sessions, and group programs for patients, families, and community members.
- Adhere to patient confidentiality & patient rights, (e.
g., HIPAA, state and organizational guidelines).
Key Responsibilities & Duties:
Navigation Care Coordination and Follow-Up:
- Conduct follow-up calls or outreach to patients/clients at designated touchpoints as directed.
- Confirm and/or review care instructions, follow-up appointments, and other recommendations
- Identify barriers to positive outcomes including social determinants of health, insurance coverage/benefits or other community support needs.
Frequent areas of concern include but are not limited to food insecurity, in home support, transportation, home repairs and accessibility, housing insecurity, access to specialty healthcare services, over utilization of emergency services, and adaptive equipment needs.
- Provide assistance and information regarding available resources and supports.
- Direct, facilitate and ensure completion of connection to appropriate resources to aid in areas of risk/need as identified.
- Promptly and accurately complete all required documentation regarding interventions, support, education etc.
Patient Satisfaction & Quality Improvement:
- Administer patient satisfaction surveys (by phone) following established protocols and sampling methods.
- Accurately record and enter survey responses in the designated database or system.
- Identify and promptly escalate significant concerns, complaints, or risk issues raised during surveys to the supervising social worker or appropriate leader.
- Support basic data collection and reporting related to patient satisfaction and service quality as requested.
Qualifications:
- High school diploma or equivalent required; associate's or bachelor's degree in social work, human services, psychology, public health, or health related field preferred.
- Experience in healthcare, social services, case management support, or customer/patient-facing roles strongly preferred.
- Basic understanding of social determinants of health, community resources, and common barriers faced by vulnerable populations.
- Strong interpersonal, communication, and customer service skills with the ability to engage respectfully and effectively with diverse patients and families.
- Good organizational and time‑management skills; ability to handle multiple tasks, calls, and follow-ups simultaneously.
- Comfort with telephone- and technology-based outreach (e.g., EMR, case management systems, survey platforms).
- Ability to maintain confidentiality, handle sensitive information, and exercise sound judgment in escalating concerns.
- Bilingual skills preferred, depending on the population served.
- Valid driver's license and reliable transportation may be required based on program needs.
- Ability to pass required background checks and health clearances.
Working Conditions:
- Primarily office or clinic-based, with frequent phone and in-person contact with patients/clients and staff.
- Some remote/telephonic work may be required for follow-up calls and surveys.
- Occasional travel within the community for outreach, home visits, or meetings may be required.
- May occasionally require flexible scheduling, including some evenings or weekends, based on patient and program needs.
