Job Description
Overview We are seeking a dedicated professional to oversee the coordination of care for our clients. This position is vital in ensuring that all aspects of client care are managed effectively and compassionately. Key Responsibilities Manage and coordinate the delivery of care services to clients. Collaborate with healthcare professionals and support staff to ensure comprehensive care plans. Monitor client progress and adjust care strategies as necessary. Facilitate communication between clients, families, and healthcare providers. Ensure compliance with all regulatory requirements and company policies. Requirements A minimum of a degree in healthcare management, nursing, or a related field. Proven experience in care coordination or healthcare management. Strong organisational and communication skills. Ability to work collaboratively in a team environment. Proficient in using electronic health records and other healthcare technology. Nice to have Certification in care coordination or case management. Experience working with diverse populations. Knowledge of local healthcare resources and services.
