(k) Match, Comprehensive Benefits Package About the Role We are seeking a dedicated Care Coordination Manager to join our team and support individuals with Intellectual and Developmental Disabilities (I/DD). In this role, you will serve as the primary point of contact for members, families, and advocates while coordinating healthcare and community services to ensure high-quality, person-centered care. This is a hybrid position with opportunities available in both Cortland, NY and Binghamton, NY . Key Responsibilities Manage a caseload of individuals with Intellectual and Developmental Disabilities (I/DD) Coordinate medical, behavioral health, OPWDD, and community-based services Develop, implement, and maintain individualized Life Plans Act as a liaison between members, families, advocates, providers, and community resources Monitor member progress and ensure services align with individual goals and needs Promote person-centered care and positive outcomes Qualifications Candidates must meet one of the following requirements: Bachelor's Degree and 2 years of relevant experience; OR Master's Degree and 1 year of relevant experience; OR Registered Nurse (RN) with 2 years of relevant experience

Ankush Pay:
