2988261089-2 Apply for this job now Job Description Kaiser Permanente seeks an LCSW specializing in Crisis & Triage Management to support members through technology-enabled behavioral health services. In this hybrid role, you will conduct rapid risk assessments, provide brief crisis intervention, and develop safety plans via phone and digital platforms. You will coordinate higher levels of care, connect patients to community resources, and document all encounters in EHR systems. Partnering with clinicians and IT teams, you'll help refine virtual workflows and leverage data to improve access, quality, and patient outcomes in a collaborative, innovation-driven environment. Responsibilities Provide crisis assessment, triage, and short-term intervention for members via phone and digital channels Develop safety plans and coordinate higher level care or community resources as needed Document assessments, interventions, and care plans in electronic health records with high accuracy Collaborate with psychiatrists, primary care, and IT teams to optimize virtual care workflows Use Kaiser Permanente digital tools to monitor risk, follow up, and ensure continuity of care Participate in case conferences, quality improvement, and data-informed practice initiatives Educate patients and families on coping strategies, resources, and follow up care Contribute clinical input to the design and enhancement of behavioral health technology solutions Required Skills Crisis assessment and intervention Suicide and violence risk assessment Safety planning and de-escalation Clinical documentation in EHR systems Knowledge of DSM-5 and behavioral health diagnoses Care coordination and referral management Telehealth and virtual care platforms Brief evidence-based therapies (CBT, DBT skills, solution-focused) Interdisciplinary team collaboration Data-informed and outcomes-focused practice
