Job Description

Job Title:
Managed Care Coordinator (Contract)
Type:
Full-Time Contract (Classic Contingent)
Location:
North Carolina (Field-Based, Hybrid)
Schedule:
Monday-Friday, 8:00 AM-5:00 PM; occasional evenings/overtime as needed
Work Arrangement:
Hybrid (1-4 days/week onsite), remote work permitted
Travel:
Up to 60% travel required Overview The Managed Care Coordinator is responsible for managing members' individual service plans and coordinating non-clinical healthcare services. This role focuses on assessing member needs, connecting members with appropriate resources, and supporting care coordination efforts without requiring clinical judgment. Responsibilities Conduct telephonic and face-to-face member assessments using standardized tools. Identify members who may require clinical intervention and coordinate with clinical care management teams. Manage non-clinical needs for members with chronic illnesses, disabilities, or complex healthcare needs. Develop and monitor short- and long-term goals with members, caregivers, families, and providers. Connect members with community resources and support services. Promote efficient utilization of healthcare services and identify opportunities for expanded member services. Travel throughout assigned territory to meet with members as needed. Required Qualifications Bachelor's degree required. Minimum 1 year of experience working directly with individuals in the target population or within community-based organizations. Equivalent combination of education and experience may be considered. Valid driver's license and ability to travel extensively throughout the assigned region. Preferred Qualifications Bachelor's degree in a healthcare-related field. Experience in care coordination, social services, community outreach, case management, or healthcare support roles. Community Health Worker (CHW) certification preferred; required within one year for applicable programs. Additional Details Not a clinical role and does not require clinical decision-making. Position is patient/member-facing. No Florida Level II Fingerprint Screening required. No access to medication and no work performed in care centers, clinics, assisted living facilities, or member homes. This opportunity is not structured as an internship. The hiring team cannot provide internship supervision or complete school-credit documentation. Location-Specific Requirement One opening supports Alleghany County, NC.
