  • 25 an hour PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Southern Colorado .
This role includes assessments, client follow-up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately. What You'll Do Manage an assigned caseload of clients Coordinate care with clients, families, caregivers, and community partners Complete intake assessments and required client visits Support caregiver scheduling and coverage needs Help address client or staff concerns promptly and professionally What We're Looking For Healthcare, case management, or caregiving experience preferred Strong communication, organization, and problem-solving skills Comfortable working independently, in a fast-paced environment, and as part of a team Work Setting This role includes working from home, attending occasional in-office meetings, visiting client homes, and traveling throughout the field.
