Job Description

Sevita is seeking a CLASS Program Coordinator to support individuals in community-based healthcare and social assistance programs. In this role, you will coordinate person-centered service plans, monitor program quality, and ensure compliance with CLASS and state guidelines. You will collaborate with families, caregivers, and interdisciplinary teams, manage documentation, and track outcomes. Responsibilities Coordinate and monitor CLASS program services for assigned participants Develop, implement, and update person-centered service plans Ensure compliance with CLASS, Medicaid, and state regulatory requirements Maintain accurate, timely documentation and case records Collaborate with families, caregivers, and interdisciplinary team members Conduct regular client visits and assessments to track progress and needs Respond to participant and family concerns and help resolve service issues Collect and analyze service data to support quality improvement Coordinate referrals to internal and external community resources Participate in training, team meetings, and program development initiatives Required Skills Care coordination Case management Knowledge of CLASS/waiver programs Regulatory compliance Documentation and reporting Care planning Client assessment Electronic health records Microsoft Office Data tracking and quality improvement
