Job Summary:

The Value-Based Care Specialist will work to improve patient outcomes and ensure cost-effective healthcare by supporting the implementation, monitoring, and optimization of value-based care initiatives. This role involves collaborating with healthcare providers, payers, and other stakeholders to ensure that healthcare services are aligned with value-based reimbursement models. The Value-Based Care specialist will also play a key role in tracking and analyzing clinical and financial data to improve quality, reduce unnecessary healthcare costs, and enhance patient care. Must have knowledge of MCO portals, HEDIS measures and EMR systems

Key Responsibilities:

Collaborate with clinical teams to identify patients who are eligible for value-based care programs. Provide ongoing support to ensure smooth implementation of care initiatives within the clinical practice, including ensuring effective care coordination and follow-up.
