Join Legacy Home Health Agency as a Field Care Coordinator and play a key role in ensuring that clients receive high-quality, reliable care in the comfort of their own homes.
Key Responsibilities:
Conduct home visits to assess client needs, ensure adherence to care plans, and provide training or oversight for caregivers. Complete and submit accurate documentation for each client visit in a timely manner. Communicate with clients and caregivers to address service-related concerns, changes in condition, or care quality issues. Respond promptly to service interruptions, urgent needs, or client concerns while in the field. Collaborate with PAS leadership to maintain care standards and regulatory compliance. Monitor and report on caregiver performance, client satisfaction, and any changes in service needs.
Job Qualifications:
May have completed two (2) years of full-time study at an accredited college or university; OR may substitute one year of full-time employment in a supervisory capacity in a health care setting for each required year of college. Reliable transportation and ability to travel locally for home visits. Excellent communication and organizational skills. Proficient in Microsoft Office and comfortable using electronic documentation systems. Bilingual (English/Spanish)
