Theraluma is seeking a compassionate, organized, and collaborative QMHP / Case Manager / Care Coordinator to join our growing outpatient mental health practice. This position will work alongside the therapist/practice owner to help clients and families navigate services, connect with community resources, coordinate care, and address practical barriers that may impact progress in outpatient therapy.

Position Responsibilities Responsibilities may include:

Provide case management and care coordination to children, adolescents, adults, and families Identify and connect clients with appropriate community resources Assist with referrals and follow up to support successful connections with outside services Coordinate with schools, healthcare professionals, social service agencies, and other providers when authorized and appropriate Help clients and families navigate behavioral health and community-based systems Assist in identifying barriers to accessing needed services Promote continuity between outpatient therapy and outside supports Complete case management and coordination documentation in a timely and professional manner Share relevant care-coordination information with the treating clinician Assist with administrative and practice-support tasks as needed Qualifications Candidates with backgrounds or credentials such as QMHP, Case Manager, Care Coordinator, Behavioral Health Specialist, or a related helping profession are encouraged to apply. Experience working with children, adolescents, families, schools, foster/adoptive families, DSS, community agencies, courts, trauma, and complex family systems is especially valued. Administrative or mental health office experience is preferred and may include assisting with general practice operations and administrative needs.
