Job Description

Resident Care Coordinator - Job Description Reports to: Director of Nursing The RCC works alongside the RN/LPN and coordinates with the Director of Nursing and Executive Director to assist with the operations of the community's assisted living program, aligning resident needs, state regulations, and company policies and procedures to provide exceptional care to residents.
Job Responsibilities:
Assist with hiring, training, supervising, and scheduling care staff members. Assisting with the caregiver scheduling and resident assignments, working within the department allowances. Assist with schedule coverage and on-call as needed. Coordinate and monitor all services relating to resident care needs through service plans. Coordinate admissions with the DON, including supervising move-ins to be sure accommodations are as desired, and care is immediately implemented. Update care and services to residents as needed through communication logs and service plans; meet with residents and family members to review service plans and charges. Provide hands-on training and support to medication aides and caregiving staff. Alert the RN/LPN when a resident has a change of condition. Conduct timely audits of the medication administration program to ensure accuracy and completeness. Address resident and family concerns in a timely and appropriate manner. Model leadership and maturity to staff; present professionally to residents and family members. Maintain confidentiality of residents, community, employees, and company Keep all documentation current and accurate (Community forms, medication documentation, medication administration forms, licensing documentation, physician orders, change of dosages, written orders to confirm telephone orders, incident reports, etc.) Communicate resident status changes. Ensure the medications room is supplied with required ongoing medications, PRN, over-the- Counter (OTC), and other medications as ordered by the physician. Coordinate medication orders and deliveries with pharmacies. Follows proper procedures on incidents or accidents. Communicate with physicians and other healthcare providers as needed. Complete an accurate count of all medications, including controlled substances per policy. Control medication room access and key assignment. Read all communication notes regarding the community between the Caregiver shifts. All other duties as assigned. Qualifications Have or obtain state certifications and licenses. Demonstrated experience supervising team members and providing training and support. Understand and follow State regulations as well as company policies and guidelines. English language required. CPR Certification. Must pass a criminal background check.
Pay:
$26.00 - $30.00 per hour Expected hours: 40.0 per week
