Make a Difference in the Lives of Others! Solutions In-Home Care is looking for a compassionate, organized, and dependable Care Coordinator to join our team! The Care Coordinator plays an important role in ensuring clients receive safe, consistent, and high-quality in-home care. This position works closely with clients, families, caregivers, the Registered Nurse, and office team to coordinate care, support caregivers, and maintain smooth daily operations.

Position Details Pay:

$25.50-$28.00 per hour, depending on experience

Schedule:

Full-time, 40 hours per week

Hours:

Monday-Friday, 8:00 AM-4:30

PM Location:

Tri-Cities, WA Work Setting:

In-person, with travel to client homes and community locations as needed Key Responsibilities Coordinate client care, admissions, care plans, and follow-up. Assist with caregiver scheduling, shift coverage, and client-caregiver matching. Build positive relationships with clients, families, and caregivers. Support caregiver onboarding, training, coaching, and ongoing engagement. Maintain accurate documentation and ensure compliance with company policies and applicable state requirements. Communicate client and caregiver concerns to appropriate team members. Assist with community outreach and referral development. Participate in the after-hours On-Call rotation as needed. Qualifications Minimum of 1 year of office experience, preferably in healthcare. Minimum of 1 year of caregiving experience, preferably in home care. High school diploma or GED required; Associate degree preferred. Strong communication, organization, and problem-solving skills. Ability to manage multiple priorities and work effectively with a team. Valid driver's license, automobile insurance, and reliable transportation. Current Washington State CNA, HCA, or NAR credential, as applicable. Current CPR/First Aid certification and Food Worker Card, as required.
