Job Description

Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC) founded in 1978, is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington State. We are recruiting for the following position: Sea Mar is a mandatory COVID-19 and flu vaccine organization Care Coordinator II - Clinical Care Team Sea Mar Community Health Centers is seeking a Care Coordinator to support patients with chronic conditions and behavioral health needs. As part of the Clinical Care Team, you will participate in daily huddles, identify patient needs, close care gaps, and coordinate care at the point of service. Key Responsibilities Participate in daily huddles and identify gaps in care Provide brief interventions for chronic illness and behavioral health support Coordinate care transitions after ER or hospital discharge Connect patients to medical, behavioral health, dental, and community resources Track and document patient progress in the EHR Support quality improvement and clinical performance measures Assist with 340B medication assistance program (site dependent) Provide excellent customer service and patient support Qualifications LPN (ambulatory care experience preferred) or
BA/BS/BSW
in health-related field + 1 year community health experience OR 4 years equivalent experience Strong communication, organization, and patient engagement skills Knowledge of chronic disease and behavioral health preferred Bilingual English/Spanish strongly preferred Valid WA driver's license and proof of insurance required
COVID-19

Starting Wage:
$23.31 per hour
Work Schedule:
Monday - Friday (Can be edited based on need) Hourly - Hourly Plan, 23.31
