OASIS Registered Nurse Position At Concordia Visiting Nurses, we are committed to providing compassionate, high-quality care that helps our patients remain safe, independent, and comfortable in their own homes. We are seeking an
OASIS RN
to join our clinical team and support the accuracy, completeness, and quality of OASIS assessments and clinical documentation. This is an in-office position and will work closely with our clinical and leadership teams to ensure OASIS documentation meets Medicare and regulatory requirements. OASIS RN
; you will be responsible for reviewing OASIS assessments and supporting clinical staff with accurate and compliant documentation.
Responsibilities include:
Reviewing OASIS assessments for accuracy, completeness, and consistency Ensuring OASIS documentation supports the patient's clinical condition and plan of care Reviewing documentation for compliance with Medicare and home health regulations Identifying discrepancies, missing information, and opportunities for improvement Collaborating with clinicians to clarify and correct OASIS documentation when needed Providing guidance and education to clinical staff regarding OASIS requirements and documentation standards Reviewing assessments for appropriate coding and clinical consistency Supporting accurate patient outcomes and reimbursement through thorough OASIS review Staying current with changes to OASIS guidance, Medicare requirements, and home health regulations Maintaining timely and accurate documentation within the electronic medical record Working collaboratively with the clinical team to promote quality, compliance, and positive patient outcomes
