Per diem role This is a multi-facility role Remote 2 weekends a month Some holiday coverage May be asked to cover for full time CE- vacations

Responsibilities:

Conduct thorough clinical evaluations for patients admitted to our skilled nursing and rehabilitation programs Develop and implement individualized care plans in collaboration with healthcare teams Monitor patient progress and make adjustments to care plans as needed Provide medical direction and guidance to nursing staff Participate in quality improvement initiatives and contribute to the development of clinical protocols Collaborate with interdisciplinary teams to ensure seamless patient care Maintain accurate and up-to-date records and documentation
