Job Description

Overview We are seeking a dedicated and compassionate Case Manager to coordinate comprehensive patient care within a dynamic healthcare environment. The ideal candidate will possess extensive experience in hospital and critical care settings, with a strong understanding of medical terminology, discharge planning, and utilization management. As a Case Manager, you will serve as a vital link between patients, healthcare providers, and insurance companies to ensure optimal health outcomes and efficient resource utilization. This role offers an opportunity to make a meaningful difference in patient lives while working within a collaborative, multidisciplinary team. Duties Conduct thorough patient assessments to determine care needs, utilizing expertise in acute care, ICU, pediatric (PICU), and emergency medicine settings. Develop individualized care plans that incorporate medical, social, and psychological factors, ensuring alignment with clinical guidelines and patient preferences. Coordinate discharge planning by collaborating with healthcare teams, social workers, and community resources to facilitate seamless transitions from hospital to home or other care facilities. Review medical documentation using CPT coding, ICD-9, ICD-10 coding systems, and manage utilization review processes to optimize patient care and resource allocation. Communicate effectively with patients and their families to explain treatment options, discharge instructions, and available social services while respecting cultural diversity. Maintain accurate documentation within EMR/EHR systems in compliance with HIPAA regulations and organizational policies. Participate in case conferences and multidisciplinary team meetings to evaluate patient progress and adjust care strategies accordingly. Monitor patient progress through ongoing assessments and coordinate appropriate interventions across hospital medicine, behavioral health, hospice care, and managed care programs. Requirements Proven experience in hospital environments such as Level II trauma centers or acute care units; ICU or PICU experience is highly desirable. Strong background in nursing or social work with knowledge of medical terminology, discharge planning, and healthcare regulations including HIPAA. Familiarity with
ICD-9/ICD-10
coding systems, CPT coding, utilization review, and utilization management processes. Demonstrated ability to work effectively with individuals from diverse cultural backgrounds in fast-paced settings. Proficiency in Microsoft Office applications as well as EMR/EHR systems for documentation and communication purposes. Knowledge of health insurance procedures, managed care protocols, and patient service coordination. Excellent client communication skills coupled with the ability to handle intake assessments sensitively and professionally. Bilingual abilities are considered an asset for serving diverse populations effectively. $25.83 - $30.00 per hour
