Full Job Description Position Overview:
ACI is seeking a qualified Counselor to provide intake services to clients in our Outpatient settings.
Responsibilities:
Provide support to individuals entering early recovery Facilitate or co-lead small groups of patients/ clients/ or significant others to increase understanding of the consumers issues or to improve coping skills and performance of activities of daily living Function autonomously and utilize appropriate judgment in the analysis and solving of sophisticated problems Through regular contact with patient/ client and provider, monitors patient/ client participation and satisfaction in all services used by consumers on their caseload, and determine if the individual is progressing and if specific services continue to be needed and are appropriate Participate in quality improvement addressing medical record documentation, quality reviews, and utilization related to clinical work performed Perform other duties as assigned Demonstrate ability to manage a caseload of individual clients Provide complete and thorough assessments of each client Demonstrate ability to facilitate educational and psychodynamic addiction and mental health recovery-based groups Complete documentation within EMR in a timely and accurate manner, including but not limited to, group note documentation, one on one sessions, etc. Demonstrate ability to check-in with clients on an individualized, as needed basis according to level of care Demonstrate ability to individualize treatment planning, and discharge planning with each client Demonstrate ability to provide crisis intervention and/or crisis stabilization when necessary Attend weekly collaborative meetings and supervision to ensure quality of care Consult with Administrator, and other staff members to ensure quality of care when necessary Comply with local, state, and federal confidentiality laws and regulations Demonstrate sensitivity and responsiveness to cultural differences Abide by ACI's and our governing bodies' code of ethics and moral values Develop & Document full assessments, treatment planning, monitor patients' clinical progress in treatment and discharge planning
