A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.

Current licensure as a Registered Nurse, Social Worker, or other licensed healthcare professional (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist). Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, ARN, RN-BC). Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute or rehabilitation hospital experience strongly preferred. Strong written and verbal communication skills in English; additional languages are a plus. Proficiency with basic computer applications (e.g., Excel, Word, Outlook, PowerPoint). Excellent organizational, time management, and analytical skills , with the ability to work independently. Collaborative mindset with a strong team-oriented approach .
