How you'll contribute Coordinate patient care to ensure effective resource utilization, quality service delivery, and compliance with regulatory requirements. Conduct comprehensive assessments, develop and implement care plans, and evaluate patient progress to support individualized needs and preferences. Facilitate care coordination and communication across the interdisciplinary team, patients, and families. Lead discharge planning efforts to promote smooth transitions, continuity of care, and cost-effective outcomes. Advocate for patients by addressing their clinical, emotional, and social needs. Support utilization review and case management functions to enhance patient outcomes and satisfaction. Promote and uphold the hospital's mission, vision, and values. Qualifications and requirements:

Current licensure as a Registered Nurse, Social Worker, or other licensed healthcare professional (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist). Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, ARN, RN-BC). Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute or rehabilitation hospital experience strongly preferred. Strong written and verbal communication skills in English; additional languages are a plus. Proficiency with basic computer applications (e.g., Excel, Word, Outlook, PowerPoint). Excellent organizational, time management, and analytical skills , with the ability to work independently. Collaborative mindset with a strong team-oriented approach .
