Job Description

Overview Rainbow Hill Recovery is an outpatient treatment program supporting adults (LGBTQIA+ and Allies) with mental health and substance use issues. Rainbow Hill Recovery is an all-inclusive, identity-driven, culturally affirming, and diverse program designed to support mind, body, spirit, and recovery from substance use disorders and mental distress. Rainbow Hill Recovery has created a nurturing environment that empowers personal growth, provides treatment at the partial hospitalization, intensive outpatient, and outpatient program levels of care (PHP/IOP/OP), and focuses on integrative, trauma informed, evidence-based, and affirming treatment. Rainbow Hill Recovery is seeking a compassionate and skilled Case Manager to join our team. The ideal candidate will be passionate about supporting the needs of individuals struggling with mental health and substance use issues. Job Summary:
Reporting to the Clinical Director, the Case Manager delivers clinical services to clients with mental health and substance use disorders; provides appropriate documentation of services; participates in training and supervision; collaborates with outreach and admissions teams; and provides milieu support. Hours would fall within Monday-Friday 9:00am-5:00pm (PST). The ideal candidate will have extensive experience providing evidence-based counseling and support to individuals struggling with substance use disorders, co-occurring mental health conditions, and related behavioral challenges. This role involves working with diverse populations, in an outpatient clinic setting and virtual telehealth platforms. Responsibilities Conduct comprehensive patient assessments including intake interviews, and diagnostic documentation utilizing ICD-10 coding standards. Develop and implement individualized treatment plans based on clinical assessments and evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, trauma-focused therapy, and behavioral health interventions. Provide individual counseling/case management sessions utilizing various modalities/therapeutic techniques tailored to client needs. Facilitate crisis intervention and crisis management to ensure client safety during acute episodes. Manage case documentation in compliance with clinical documentation standards, EMR/EHR systems (Kipu), HIPAA regulations, and clinical/ASAM/medical necessity terminology requirements. Collaborate with multidisciplinary teams including therapists, medical professionals, and support staff to coordinate care for clients with complex needs such as co-occurring disorders or developmental disabilities. Maintain accurate medical records and ensure timely updates of client progress notes while adhering to clinical ethical guidelines. Engage in ongoing research and professional development to stay current with best practices in addiction counseling, mental health treatment planning, ICD coding updates, and telehealth therapy modalities. Develop and implement individualized care plans that align with clinical guidelines, including discharge planning and utilization review. Monitor patient progress through ongoing communication with clients, families, and treatment team to adjust care plans as needed. Document all case management activities thoroughly and accurately in EMR/EHR systems while adhering to HIPAA privacy standards. Experience Prior experience working with diverse populations, including addiction, mental health, ASD, and LGTBQIA+ is highly desirable. Demonstrated experience in evidence-based practices such as cognitive behavioral therapy (CBT), trauma-focused therapy, applied behavior analysis (ABA), grief counseling, PTSD care, gender issues counseling, and addiction counseling. Excellent client communication skills and experience working with individuals with co-occurring disorders or developmental disabilities is preferred. Familiarity with clinical documentation standards using EMR/EHR systems (Kipu); proficiency in
ICD-10/ CPT
coding; experience conducting assessments; case management skills; crisis management capabilities; telehealth experience are advantageous. Knowledge of discharge planning processes and utilization management within managed care frameworks. Knowledge of medical terminology related to mental health conditions; understanding of medical records management; experience working within inpatient or outpatient settings; familiarity with HIPAA regulations and clinical ethical guidelines are essential. Pay:
$28.00 - $32.00 per hour Expected hours: 32.0 - 40.0 per week
