POSITION OVERVIEW
The primary purpose of HHCLA's Housing for Health (HFH) program is to provide permanent, affordable housing linked to appropriate services for homeless individuals who are frequent users of the Department of Health Services system. The Clinical Case Manager I works directly with program participants and develops, arranges and coordinates treatment services. The Clinical Case Manager I works to improve the health of homeless individuals by assisting with the provision of stable housing, development of a comprehensive treatment plan and facilitating linkages to medical care, mental health care, alcohol/substance use treatment and other supportive services. Once the individual has obtained permanent housing, the Clinical Case Manager I will assist the client in maintaining their housing. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy.PRINICIPAL DUTIES & RESPONSIBILITIES
- Under the supervision of a Program Manager, Clinical Case Manager 1 conducts comprehensive clinical assessments and provides intensive case management services.
- Under the supervision of a Program Manager, Clinical Case Manager 1 develops individualized treatment plans with measurable goals and objectives and assists participants in achieving their desired outcomes.
- Work with a with a diverse and marginalized client caseload of justice-involved clients experiencing barriers including a history of homelessness, complex trauma, medical, mental health, and substance use.
- Maintain weekly contact with all clients and document contact in the county approved database.
- Coordinates, prepares and maintains required charting and documentation in a timely and thorough manner within program guidelines.
- Develop relationships with representatives in other agencies to support individuals in attaining services such as housing, mental health care, medical treatment, financial assistance, legal advocacy, and other community resources.
- Assist with preparation of housing applications and other required documentation.
- Networks and collaborates with housing providers, landlords and property managers to develop housing stock for program, which includes temporary and permanent housing alternatives.
- Performs home visits within scheduled time frame, assesses for needs on a continuous basis, and refers clients to services within their community.
- Maintains familiarity with
HHCLA/ HFH
policies and procedures and other applicable regulations.- Assists in identifying and securing resources for the HFH program.
- Provides support and link clients to Electronic Homeless Programs, including CES and HMIS Systems.
- Participate in court hearings and submit court progress reports.
- Provide medication assistance and tracking.
- Under the supervision of the Program Manager, the Clinical Case Manager provide clinical intervention services, crisis intervention, conflict resolution and continually monitor at-risk/high risk situations that require clinical interventions.
- Conducts advocacy services and ensures clients receive all entitled benefits.
- Participates in debrief sessions, weekly supervision, clinical supervision, department and agency-wide meetings, training and development opportunities as determined appropriate by the supervisor.
- Submits weekly/monthly reports, such as mileage, weekly schedules, quarterly training reports, corrected audit findings, and status of unhoused individuals.
- Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others.
- Ensures services are delivered using the "Whatever It Takes" model by consistently applying client choice, harm reduction, trauma-informed care, Housing First, integrated care, and psychosocial rehabilitation principles.
- Able to serve a diverse vulnerable client populations and communicate effectively in a culturally competent manner.
- Able to work in a multi-disciplinary team environment, utilizing professional, open and respectful communication. Maintains a solution focused, strengths based and client centered approach with managers, co-workers and clients of HHCLA.
- Ensures performance management standards are met through ongoing supervision addressing barriers as they arise.
- Maintains client records according to HIPAA and adheres to all client confidentiality requirements and standards.
- Complies and maintains familiarity with agency and program policies and procedures and other applicable regulations.
- Attends and participates in supervision, staff meetings, trainings, conferences, workshops, and special projects to promote professional development.
- Models HHCLA approach, mission and core values in all communication, correspondence, community events, coalitions and advocacy efforts.
- Reports to work on time and maintains reliable and regular attendance.
- Maintains current licensing and/or certification as required for position.
- Other duties as assigned.
QUALIFICATIONS & SKILLS
REQUIRED:
Education and Work Experience:
- Bachelor's Degree in the social services/human services field.
- 2+ years' experience working in housing and with culturally diverse populations, including co-occurring and homeless.
Knowledge and Skills:
- Familiar with the principles of Harm Reduction, Motivational Interviewing, Trauma Informed Care, Housing First and Integrated Care
- Experience working with culturally diverse communities and vulnerable populations
- Ability to effectively present information and respond to questions from clients, office personnel, other employees and the general public
- Ability to work in fast-paced, high energy environment
- Ability to define problems and use critical thinking skills to identify solutions
- Ability to work independently and with others
- including other employees, clients and members of the public
- in face-to-face and telephonic contexts
- Ability to multi-task, establish priorities and manage time, meet deadlines and perform duties under time constraints
- Ability to be flexible and adapt to changing work demands
- Maintain high level of concentration and attention to detail for extended periods of time
- Ability to respond effectively to sensitive inquiries or concerns
- Maintain a high level of ethical and professional standards in accordance with agency policies
- Computer literate (basic working knowledge of Microsoft WORD, Excel, and Outlook) and data/electronic records
- Must possess a valid California Driver's License, properly registered vehicle and insurance
Certificates, Licenses, Registrations:

- Bilingual
- Familiarity with HMIS database desired.
- Knowledge of non-violent crisis intervention.
