Casa Recovery is seeking an experienced Case Manager (Drug & Alcohol Counselor or Social Worker) to join our multidisciplinary team at our outpatient treatment center. Since 2009, we have been a leader in treating mental health and co-occurring disorders. Unlike high-volume facilities, our milieu is intentionally small, ensuring you have the time and resources to provide truly individualized care. What You'll Do As a vital link between our clients and their recovery goals, you will:
One-on-One Advocacy:
Conduct regular individual case management sessions to assess client progress, identify barriers to recovery, and provide personalized clinical support.
Discharge & Aftercare:
Facilitate comprehensive discharge planning and coordinate aftercare to ensure clients have a solid roadmap for long-term sobriety and mental wellness.
Clinical Collaboration:
Partner with therapists, psychiatrists, and holistic practitioners to ensure a unified approach to the treatment plan.
Group Leadership:
Facilitate engaging group sessions and educational workshops.
Treatment Planning:
Develop and update comprehensive treatment plans and maintain precise records via Kipu EMR.
Family Support:
Coordinate communication with families and external stakeholders to support a healthy home environment and a seamless continuum of care.
Qualifications Certification:
Must be a certified drug counselor in California (CCAPP, CATC, CADC, etc.) OR a qualified Social Worker.
Experience:
2+ years of professional experience in case management or counseling.
Clinical Knowledge:
Strong exposure to mental health disorders and evidence-based treatment modalities.
Tech Savvy:
Proficient with Windows, Google Workspace, and electronic medical records ( Kipu experience is a major plus! ).
Availability:
