Job Overview Alternative Homes for Youth (AHFY), is a residential treatment center in Greeley, Colorado, serving adolescent males ages 15-20 involved in the juvenile justice system. We are seeking a passionate and proactive Clinical Case Manager to join our dynamic team. In this vital role, you will provide case management services designated by the treatment team and governing agencies. Case management services include client support, medical coordination, medication management, organization and coordination of various documentation.
Responsibilities:
- Adhere to all AHFY written policies, procedures and practices.
- Compliance with all federal, state and local laws (including licensing rules and regulations) regarding child welfare, mental health services, alcohol and drug abuse
- Attend facility staff meetings
- Coordinating new client arrivals and intakes
- Provide support, information and transportation to all court and ARD appointments, or attending via Zoom/video chat as allowed
- Schedule and track all psychiatric appointments
- Coordinate payment, schedule, and send questions for youth polygraphs.
- Accompany youth when sex offender registration is required
- Assist youth with clearing any active warrants
- Obtain youth signatures on all required documentation
- Ensure Medicaid and/or Insurance coverage is in place.
- Conduct exit interviews
- Communicate accurate and complete information to client managers, caseworkers, family, and other designated individuals.
- Maintain youth phone contact list
- Manage psychiatric documentation between psychiatrist, caseworker and legal guardian
- Arrange with Clinical Director to attend job-related conferences and workshops to actively continue professional development.
- Assist with crisis management for all youth.
- Ensure youth receive initial medical, dental, and vision appointments within required timeframes, and then as needed by youth
- Responsible for ordering and packaging youth medication and the review of medication error forms
- Responsible for training new staff on EMARs and medication groups.
- Respond to all internal medical reports and ensure youth medical needs are met.
- Schedule and transport youth to and from medical and dental appointments
- Conduct UAs, BAs, and searches as needed.
- Overseeing QMAP compliance Assist with other responsibilities as assigned.
Pay:
$55,000.00 - $70,000.00 per year
