, you will play a crucial role in coordinating and optimizing healthcare services for clients to promote their well-being and ensure they receive the necessary medical care according to their individual health needs. This position involves working closely with patients, healthcare providers, and insurance companies to facilitate effective healthcare solutions and navigate complex healthcare systems. Responsibilities Assess new clients and gather all necessary health and personal information to plan appropriate care. Develop, implement, and review personalized care plans in coordination with healthcare professionals. Regularly monitor and evaluate the effectiveness of care plans, making adjustments as needed. Advocate for clients' needs with healthcare providers, social services, and other stakeholders. Maintain accurate records of clients' treatment plans, medical records, and progress reports. Educate clients and their families about their conditions and about how to access various health services. Ensure compliance with all federal and state regulations regarding medical case management. Required Skills Strong communication and interpersonal skills. Proficiency in case management software and medical record systems. Ability to handle sensitive information confidentially. Knowledge of healthcare regulations and policies. Strong organizational skills and attention to detail.
