Job Description
CAN Community Health seeks a Medical Case Manager 1 to coordinate care for patients living with HIV and related conditions. In this role, you will complete biopsychosocial assessments, develop individualized care plans, and link clients to medical, behavioral health, and social services. You will monitor treatment adherence, conduct follow-up contacts, and document all services in electronic health records. Working with an interdisciplinary team, you will advocate for clients, support benefits and medication access, and promote education and prevention to improve health outcomes in underserved communities.
