Job Description

Overview We are seeking a dedicated and detail-oriented Clinical Intake Liaison to serve as a vital link between patients, families, and healthcare providers. The ideal candidate will possess comprehensive knowledge of hospital operations, clinical case management, and patient assessment procedures. This role offers an opportunity to contribute significantly to patient care coordination, ensuring seamless intake processes across diverse medical settings. The Clinical Intake Liaison will play a crucial role in facilitating effective communication, managing medical records, and supporting discharge planning efforts to optimize patient outcomes. Duties Conduct thorough patient assessments upon admission, including vital signs, medical history review, and initial clinical evaluation. Collaborate with healthcare professionals across departments such as ICU, emergency room, hospital medicine, and behavioral health to coordinate patient intake and care plans. Manage electronic health records (EHR) and medical documentation accurately while ensuring compliance with HIPAA regulations. Facilitate communication between patients, families, and clinical teams to clarify treatment plans and address concerns promptly. Assist in discharge planning by coordinating post-acute care services, hospice care, and home health arrangements. Support utilization management by reviewing diagnostic evaluations and ensuring appropriate resource allocation for inpatient and outpatient services. Maintain detailed records of patient interactions, clinical case management activities, and intake procedures to support quality assurance initiatives. Qualifications Active RN or LPN license in good standing At least two years of recent experience in home health, discharge planning, or intake coordination preferred Knowledge of Medicare home health eligibility and required documentation Strong communication skills and the ability to maintain positive relationships with referral sources Proficiency with EMR systems and intake tracking tools preferred Strong organizational and analytical ability Join our team as a Clinical Intake Liaison to ensure smooth patient transitions through comprehensive intake processes that uphold the highest standards of care and operational excellence!
Pay:
$49,304.77 - $59,377.79 per year
